· Develop monthly quality assurance reports.
· Lead nutrition-at-risk (NAR) meetings and provide clinical nutrition interventions and care plans.
· Complete monthly quality assurance audits to ensure regulatory compliance for state/federal survey preparedness.
· Participate as an essential Interdisciplinary Team Leader within each account.
· Communicate with facility representatives for coordination of care and services.
· Complete nutrition assessments and MDS sections · Participate in weekly team calls and complete QA data.
MINIMUM QUALIFICATIONS: · A minimum Bachelor of Science degree completed.
· Registered Dietitian as per Commission on Dietetic Registration · Registered Dietitian State Licensure · Valid Driver’s License · Electronic medical record (EMR) experience (PCC/Matrix) is preferred but not required.
· Experience in LTC and managing multiple LTC buildings is a plus but not required.
New RD/DTR graduates are welcome to apply! We will provide an in-depth training program to be LTC ready.
