Social Services Director

If you are a people person with a desire to assist others with their social needs, this may be the position for you! The successful candidate will be required to work closely with residents, their families and the interdisciplinary team to ensure that the needs of the residents are met.
Schedules and participates in care plans; assists with admission and discharge planning services; monitors and tracks behavioral issues and any other needs of residents related to social needs.
Excellent time management, communication and writing skills are a requirement.
