Job Summary:¸ Oversee and provide therapeutic services to a specialized population of elderly and other vulnerable populations in need within the office’s service area.
This is a part-time position up to 18 hours per week.
Duties/Responsibilities: 1.
Offer direct service to clients within the Elderly Guardianship program including clients with developmental disabilities and mental illness.
2.
Provide “on call” service for clients in the Elderly Guardianship program.
3.
Receive clients from Eldercare Options Coordinator and assist with the court process as needed and directed.
4.
Perform project required record keeping in a timely manner consistent with the funding sources and COA requirements.
5.
Offer assessment and referral services to elderly clients according to their specific needs.
6.
Make monthly visits to clients.
7.
Attend Care Plans, annual staffing and all other meetings for clients.
8.
Work closely with doctors, nursing home, attorneys and funeral homes as needed.
9.
Serve as the agency representative with other community organizations serving the elderly, as directed by the supervisor.
10.
Work in a cooperative manner with the supervisor and administrative office staff in the enhancement and/or development of Elderly Guardianship program.
12.
Attend necessary and required training to maintain and implement current knowledge of program and elderly services.
13.
Report incidents of suspected abuse and neglect to all appointed entities as listed in procedures.
Required Knowledge/Skills/Abilities, Education and Experience: Bachelors degree in Social Work or human services field 3-5 years of direct service experience in related field (social work and/or human services Ability to accept instructions and apply them in a logical and responsible manner and to make productive us of unsupervised time.
Sensitivity and respect for populations of individuals who may be considered marginalized regarding their socio-economic, emotional, physical and spiritual backgrounds and the ability to work within a team concept and the philosophy of the agency mission.
Dependable car, valid drivers license and current auto insurance in compliance with all state and federal regulations Maturity, flexibility, dependability and strong organizational skills.
Related Job Requirements: In state travel required with self-insured vehicle; accessibility outside normal work hours; driving imperative position requires meeting all criteria set forth by our auto insurance carrier.
