GENERAL DESCRIPTION Are you looking for a meaningful job with perfect hours? MRI has a job for you! Under general direction of the Director of Community Day Services, Community Day Services Case Manager/QIDP provides training to staff and to individuals served in Community Day Services on a daily basis.
The Case Manager/QIDP directs implementation of the Person Centered Plan through authoring Implementation Strategies and monitoring of methods employed, coordination of services and documentation of results; facilitates enrollment and disenrollment procedures.
The hours are Monday through Friday, 8:00 am to 4:30 pm! ESSENTIAL JOB RESPONSIBILITIES Qualified Intellectual Disability Professional facilitates the development of the PCP or implementation strategy for each individual incorporating the principles of normalization, choice, community inclusion, self-empowerment, active treatment, least restrictive environment and functional skill development.
Directs the implementation of the Implementation strategy or PCP as recommended by the IDT.
Monthly re-evaluates objectives and training methods; recommends changes and modifications to the IDT as needed.
Maintains ongoing communication with members of the IDT.
While serving as a role model and encouraging independence and decision-making skills, provides guidance to the CDS trainers in providing active training in activities of daily living, recreation/leisure, community integration, vocational, safety and self-advocacy as presented in each individuals written PCP and Implementation Strategy.
Assists the MCDS with the training and development of an effective and efficient staff: Establishes and communicates performance standards and objectives.
Monitors operations and staff, and when necessary, recommends changes in methods, procedures, structure, and additions and changes in personnel to secure optimum utilization of resources.
Communicates with staff regarding current problems, changes, and new developments in the agency by conducting periodic meetings.
Administers all policies and procedures as developed by MRI; communicates to staff; interprets as necessary, and confirms compliance.
Develops manager and/or supervisor skills, particularly in the management techniques of planning, organizing, leading, and supervising through continued coaching and feedback performance, specifically in relation to the development and implementation of curriculum in CDS.
Works with the CDS trainers to assure they provide and assist in the provision of direct training to persons served based on a written implementation strategy and assure implementation of all procedures as recommended by the IDT while serving as a role model and encouraging independence and self-directed decision-making skills.
Maintains positive person-centered communication with other programs and agencies, i.
e.
, CLS, Prairieland Service Coordination, Social Security, and Public Aid by timely, comprehensive and professional communication to facilitate coordinated service delivery.
Conducts and/or obtains assessments on a regular basis in order to identify needs and preferences of persons served, recommends services, and evaluate effectiveness of the PCP.
Provides training and assistance in the following areas to persons served to assist and support achievement of maximum levels of self-sufficiency and independence in their natural environments.
Budgeting/money management Grocery shopping (including adherence to dietary restrictions) Leisure skills/activities Personal care needs (health, hygiene, grooming) Housekeeping and care of personal belongings Health care (including follow-up) Safety Medication training Housing procurement Access to MRI and non-MRI Emergency Services Vocational Trains and assists individuals in linkage to Social Security, Public Aid and Decatur Housing Authority, including procuring all necessary documentation needed to apply/re-certify.
Provides and monitors the provision of vocational training to individuals served while supporting the progression of work skills and habilitation efforts in the individuals work setting, and be responsible for: Establishing work related goals/objectives based on individual needs and preferences.
Recording the data/information pertaining to work related goals and objectives.
Recording information on daily count sheets and submitting to Case Management Administrative Aide (CMAA) on a daily basis.
Providing supervision and assistance to persons served eating lunch in the cafeteria.
Qualifications JOB QUALIFICATIONS A Bachelors Degree in human services or related field.
QIDP certification preferred, but will train the right person to assist them in becoming QIDP-eligible.
One year experience with the developmentally disabled and/or DD/MI.
